*Please remember that parents and guardians must create a profile for themselves first BEFORE signing a waiver for their minor.
Steps for Filling Out a Waiver for You and Your Family!
*Pro Tip – create a profile for yourself before your child!
STEP 1
Scroll down until you see “Create Account” and click it.
STEP 2
Use an email for you (not your family member yet!) and a password.
STEP 3
Approach will send you an email from noreply@tilefive.com with your verification code. Please check your spam for that email address.
STEP 4
Click on the person icon at the top right.
If you need to sign a waiver for a minor, please use Steps 5&6. If you are just signing a waiver for yourself, skip to Step 7.

On the left side click “Household”
STEP 6
Click “Add A household Member” and add in your household member’s information
STEP 7
Now click Sign Waiver and it will give you the option of who to sign a waiver for. You can sign for yourself or the minors in your household.
